TEACHERS/ADVISORS: If you are bringing more than 1 team to the MN State Envirothon, please complete this form for each team that is attending.

LUNCHES: Lunches and beverages will be provided for all attendees. However, we cannot accommodate all the potential dietary needs and wishes of every event participant. If any team member or advisor has special dietary restrictions, they should plan on bringing their OWN lunch to the event. We also ask that everyone be respectful, and clean up after yourselves.

ALTERNATE STUDENTS:  Alternate Students listed MUST have competed at the Area competition. Alternates are listed as a means to substitute an original team member, if an emergency arises. We encourage you to list up to 2 alternates if available. However, we do request that alternates only attend if absolutely necessary.
