Tennis Titans - Registration Form
WELCOME TO IMPACT TENNIS ACADEMY 

We are excited to welcome your child to our Tennis Titans Program at Impact Tennis Academy! This class is designed for kids ages 12–15 who are new to tennis or building on their beginner skills. It’s a great way for young players to learn the game, improve their technique, and have fun on the court.

Here are a few important details to keep in mind:

  • 🎾 Rackets Provided – If your child doesn’t have a racket, no worries—we’ll provide one.

  • 💧 What to Bring – Please send your child with a bottle of water to stay hydrated.

  • 👚👕 Dress Code – Sport/Comfortable Attire - Short breathable pants/skirt (no jeans)
  • 👟 Proper athletic tennis shoes are required (no Crocs or sandals). 
  •      Payment: Payment should be made each by Monday. 

  • ⏰ Punctuality – Arriving on time ensures your child can take full advantage of each session.

  • 📅 Cancellation – If your child is unable to attend, kindly inform the coach in advance.

  • 📍 Location – Classes are held at The Balmoral Club (Sandford Drive).

In this program, players will learn all the fundamentals of tennis—forehands, backhands, volleys, and an introduction to serving—while developing coordination, discipline, and teamwork. Most importantly, they’ll gain confidence and enjoy the game in a positive, encouraging environment.

MEDIA RELEASE WAIVER

I understand that these images or recordings may be used by Impact Tennis Academy for promotional, documentation, and reporting purposes, including use on the Academy’s website, social media platforms, and official printed materials.

I also acknowledge that once posted online, these images may be viewed and shared by the public.

I hereby release and hold harmless Impact Tennis Academy, its employees, agents, and representatives from any and all claims, demands, injuries, damages, or legal actions arising out of or resulting from my child’s participation. This includes, but is not limited to, any loss or damage to personal property or any personal injury, whether caused by negligence or otherwise.
