In an effort to streamline the information gathering process, you will be completing the following form in lieu of the traditional enrollment paperwork.  This form includes all of the information you are accustomed to providing to your child's school at the beginning of each school year.

Please complete ALL required fields for each section of this form.  This information will make up your child's official record at MST and be entered into the Manchester School District Data Management System (Aspen).  This form could take 30 minutes to complete and will need to be finished once it is begun.  Questions about the information requested on this form should be directed to tcolbroth@mansd.org.
