Chaperone(s) $35.00 (at least one required per school in addition to the troupe director. For groups of 11 or more, 1 chaperone for each additional 10 students)

**Changes to any portion of your application or any other form will be assessed a fee of $25
per change. Kentucky Thespians reserves the right to refuse any change.
