• Throughout the semester, we will be working on on-camera acting, practicing audition techniques on camera, and engaging in projects where performances will be recorded for the end-of-semester showcase and for the children's learning process. Kindly read and sign the consent form online, granting permission for photography and video recordings.  

I, __________________________________[Parent/Guardian's Full Name], hereby grant permission for my child, _______________________________[Child's Full Name], to participate in the acting classes provided by Star On!! The Acting Studio. This consent form covers the use of photography, video recordings, and printed promotional material featuring my child during acting classes, rehearsals, performances, or related activities for promotional and educational purposes.

I understand that photography, video recordings, and printed material may be displayed on educational presentations (from our studio), studio showcases, bulletin boards, studio newsletters or emails, classroom displays, and our studio social media and/or website to showcase the students' achievements.

I, as the parent/guardian, acknowledge that I will not receive any financial compensation or royalties for the use of my child's image or likeness in the aforementioned ways. I also understand that Star On!! The Acting Studio will take reasonable measures to protect the privacy and safety of the students. All photographs, video recordings and printed material will be used responsibly and will not be shared with third parties without proper consent.

I further acknowledge that I have the right to withdraw my consent at any time, and in such a case, I will promptly inform Star On!! The Acting Studio in writing.

This consent form will remain valid until the end of my participation and enrollment in Star On!! The Acting Studio, unless otherwise specified.
