Class of 2025 Graduation Celebration Registration
 Saturday, June 7th, 2025 

10:30PM - Doors Open  
11:00PM - Doors Lock  
Sunday, June 8, Students Released - 4:45-5:00AM (NO STUDENT CAN LEAVE EARLY UNLESS IT HAS BEEN PREARRANGED WITH THE PARENT CLUB AND CONFIRMATION HAS BEEN GIVEN VIA EMAIL.) 

In order to attend the FHHS Class of 2025 Parent Club Graduation Celebration please complete the following:

1. Provide your contact information  
2. Your attendance fee must be paid in advance for your child to attend the Graduation Celebration.  
*Email fhhsclassof2025@gmail.com if you need to check if your parent club dues have been paid.  
3. You must have a Waiver on file for Amp Up.
4. You and your child must read and sign the Consent & Notification portion of the Google Form.  
5. If you, your business, or a relative would like to help sponsor the event, please complete the Sponsors Portion of the form and submit a donation check. 
6. Make checks payable without fees to:  FHHS Class of 2025 Parent Club and mail c/o Jamie Rader 257 Towers Creek Place St. Charles MO 63304  or PayPal with additional fees to:  @Fhhsclassof2025  *please note your child’s name on your payment
7. Submit the Google Form and submit the $200 Parent Club dues (if not already paid) to the above. 

**Registration Deadlines - 
Google Form completed by May 17 - Deadline to register in order to avoid $40 late fee (even if $200 dues have been paid).  
Google Form completed by May 24 - Final registration deadline. Your $40 late fee must be included in order for your registration to be complete.

CONSENT & NOTIFICATION - please read & have the parent/guardian date and type in their name.  

I/We will not hold Francis Howell High School, Board of Education, Administration, Faculty, Parent Club Sponsors, Parent Volunteers, AMP Up Event, or the entertainment companies liable for any injury while participating in events at or traveling to/from the FHHS Graduation Celebration.  Activities may include but are not limited to: laser tag; arcade games; Axe Throwing; ropes course; virtual reality games, Go Karts, and other activities. 

 I/We give permission for medical treatment.  In case of a critical emergency, calls will be made to 911 or the appropriate emergency service and the parents/guardians.  The cost of medical attention and ambulance is the responsibility of the parents/guardians. 

 I/We give permission for participant’s photo/image to be taken and posted on website for graduates and Parent Club use. 

 I/We understand that smoking, alcohol, any controlled substances, or anything deemed to be a weapon will not be allowed at this event.  I/We understand that if I am under the influence or in possession of alcohol, controlled substance, or weapon at this event, the proper authorities will be notified.  My parents/guardians will be notified if it is necessary to leave the event or if I register and don’t attend.  If I have been asked to leave, if I choose to leave, or if I don’t attend for any of the above-stated reasons, I understand that I may not be eligible for Attendance or Grand Prizes.
