Thank you for your interest in bringing a Lotus Blossoms 2017 artist to your school!  This survey will help us match your school schedule and artist preferences with our artist availability.  Matches are on a first-come, first-serve basis based on the order of completed surveys are received. Survey should take 3-4 minutes to complete.

Included in this survey is information on each Lotus Blossoms artist, an artist preference sign-up, and a brief questionnaire about your school's Fine Arts classes. Please note, Lotus staff intends to provide follow-up questions to better understand your school and how the arts play a role in your students' education.
