THIS FORM IS FOR YOU TO CONFIRM THAT YOU AND YOUR CLUB UNDERSTAND WHAT TO DO IN ORDER TO BECOME AN OFFICIAL CLUB! THIS IS THE NOT THE APPLICATION TO BECOME AN OFFICIAL CLUB!

In order to be an official club at Independence, you must FILL OUT THIS GOOGLE FORM, and send in a full club application.

A full club charter application includes:
-Club Charter Application
-Club Budget Plan
-Club Constitution (not included in the link, your club must make your own)
-Memorandum

To submit your full application, you have the CHOICE of either:

1) Printing the application out, filling it out, scanning it, and emailing it to independence.asb@gmail.com with the subject as "CLUB NAME - CHARTER APPLICATION 2020-2021"

2) Printing the application out, fill it out, and dropping it off at N-01 to Debbie's mailbox.

3) Picking up the application at N-01, fill it out, and drop it off back at N-01 to Debbie's mailbox.


If you do not have access to a printer and/or cannot come to campus to get an application packet, please contact us by Instagram @indyactivities or by email to independence.asb@gmail.com

Please fill out your club charter application as if we were still in school on campus! As for advisor signatures, please just write down your advisor's name and email on the application, and ask your advisor to send a digital signature to you to add it onto the document! There are apps such as DocHub and Kami, that offer features to add a digital signature onto documents.
