Will the parents participate in the programming we provide in it's entirety? For example, during a parent and child story time the parents stay with the children.

(We can schedule one staff member only if parents are included in the lesson and will not leave their child at any point.)

If this form is submitted within 24 hours of the event a $50 late notice fee will be added to your invoice.

50% of the total invoice is due as a deposit following the receipt of you emailed quote. The 50% deposit should be collected within 72 hours of your confirmation email or our services may be cancelled. In the event that services may be cancelled you will be notified.

Sliding scale pricing may be available inquire if needed.
Staffing Ratio
If there are ANY infants present then AN ADDITIONAL caregiver must be added for every 2 infants present regardless of how many staff are there to provide care for non-infants.

Youth are grouped ages 0-5, 5-10, 10-15, and 16-17 A staff member is needed for each age group.

If children in the program demonstrate the inability to self-regulate and need more attentive care the staffing ratios may be adjusted to compensate for this factor. We will discuss and agree to this before staffing in such situations.

The space we create:
Each Play Cousins Collective Space is created and run by the the Innovative Youth Advocate Team. These team members consist of well trained Parents, Aunties and Uncles who have completed a vetting process including a background check, interview, and Play Cousins Collective training. They have committed themselves to building the village needed to keep our children safe, strong and successful in today’s world. Their innovative ideas lend to STEM based curriculum, creative and educational games as well as joyful storytelling and artistic experiences for youth.

Because we are building a family and a village, the IYA team members are invited to bring their children to work engagements. These children also set the tone for positive behaviors, a love for learning and safe and fun play. Children brought by our team members in no way influence your invoice, and children invited are kept well under state regulated child-care ratios. Their behavior is monitored more closely as they are held to our standards as guests, with the knowledge that they will not be invited further, if poor behavior persists.

Youth who participate in Play Cousins programming will learn effective communication skills, self-regulation techniques, and respect for self and others. Our programs are designed to support and enrich the social and emotional intelligence of children. As we problem solve by asking questions and testing hypothesis, we will perfect our own scientific theories and ideas about the world we live in. Self love is the current running through all of our programming. Every Play and Learn lab is built to teach children how important and amazing they are. It is our duty to affirm that anything they aspire to is possible with hard work and imagination.
Maintaining Safe Spaces:
If you would like to recruit volunteers to aid in infant care or help facilitate programming we require our childcare guidelines be given to each volunteer before the event and that all volunteers have background checks completed. If volunteers have any child-related or sexual offenses they are not permitted to serve Play Cousins Collective. If they have committed a violent offense within the last 10 years they are not permitted to serve with Play Cousins Collective. Volunteers do not impact the staffing ratio and we will need to send the correct amount of staff per children.

Each Play Cousins care space is child-proofed and the children are consistently well supervised. At least one staff member on site is trained in CPR and First Aid. Understanding that all precautions have been taken to keep your child healthy and safe please note that accidents at times arise. The undersigned assumes all risk of injury or harm to the child associated with participation in activities lead by Play Cousins Collective and agrees to release, indemnify, defend and forever discharge Play Cousins Collective and it's staff, employees, and agents of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of death, injury, loss or damage to the child, or by the child, howsoever caused, arising or to arise by reason of or during the child's participation in activities lead by Play Cousins Collective.

By submitting this document you are also stating that you will provide our staff with a space that is suitable to provide children’s programming. This means that smoke alarms, and fire exits are provided. It also means that the space is secure, meaning people are unable to enter the space without being checked in and that people are unable to walk into the space directly from the street without encountering a locked door or security guard. By signing this document you are stating that the space you provide Play Cousins Collective with will have all outlets covered with safety plugs. The floor will be covered in carpet or a 5'0"X6'6" safety rug will be provided. The furniture cannot be made of glass and if possible should have rounded edges. Doors in the space you provide must be able to close and preferably lock, for the security of the children in our care. Children must be provided 3 feet of space. Meaning if you have 15 children the room must be at least 45 ft in area. Outdoor space provided must have mulch or grass and also be secure.

Invoicing:
-Checks should be addressed to Play Cousins Collective and sent to

-Pricing includes one CPR and First Aid Certified Innovative Youth Advocate on sight at all times, supplies, lesson planning, administrative costs, toys and other needed materials.

-Due to the nature of programming and staff reserving work days to assist with events, contracts and pay schedules will not be adjusted for cancellations made within 28 days of the event.
. If you would like to reschedule another invoice will be prepared.

-Cancellations will not be accommodated, however if you would like to reschedule contracted service times 29 days or more before the date of service you may do so.

-Rescheduling is also allotted for weather cancellations in the event that our staff cannot safely travel to the job site. (Business and school closings may be used as a reference. This does not include weather cancellations where inside provisions were not made as a backup to inclement weather.)

-If staff must be called in to adjust for the amount of children attending the program you will be billed separately $20 an hour for each additional staff member. If this happens on more than 3 occasions we will revise and re-sign the invoice to reflect the amount of staff needed moving forward.

-If no children are in need of care and programming during the event that we have staffed, our staff may be released after 1 hour. We require payment for 2 hours of service (or the full time of the contracted event if less than 2 hours) to compensate them for clearing their schedules.

-You will be sent a confirmation email once we staff your event. The email will state the amount to be invoiced based on the submission of this form. 50% of the total invoice is due as a deposit following the receipt of you emailed quote. The 50% deposit MUST be collected within 72 hours.

-Staff are instructed to leave at the end of their shift, but additional hours will be charged to the vendor if staff are requested to stay beyond the agreed upon time.
