St. Clair TNR & Rescue Adoption Application

In order to be considered for adoption, you MUST:
* be at least 18 years of age.
* have a valid ID with current address.
* provide your current landlord's name and phone number (if applicable).
* have or select a veterinarian/animal hospital in which to take your pet.
* be the person responsible for the pet.
* meet the adoption criteria.

-An adoption will NOT be allowed if you have violated humane treatment of animal provisions.  
-An adoption application will NOT be considered if it is not fully completed.

-Once your application is APPROVED, someone will be in contact with you.  All volunteers work full time as well.  
 Please be patient.

I agree to provide healthy food, fresh water, appropriate shelter, much love, timely medical care, heart worm preventative monthly, flea and tick prevention monthly, yearly vaccines, and for any other needs my new companion animal may have for the duration of his/her life. *
I agree that my new companion animal will not be an outdoor animal.  If taken outdoors, my new companion animal will be appropriately placed on a leash and will be in my company. *
I will NOT declaw my new kitten/cat. *
If for any reason I cannot afford appropriate medical care, I will contact St. Clair TNR & Rescue and return my companion animal to them. *
If for any reason I cannot keep my companion animal, i will return him/her to St. Clair TNR & Rescue.  (We do not judge.  We simply want the BEST for all companion animals and families.)
