This form is for anyone who wants to apply to take part in a Crooked House or Kildare Youth Theatre project. Note that it is not a Membership Form for Kildare Youth Theatre. That's a separate one and can be found here

You fill in this form if, for example, you want to apply to audition for a play (like the NT Connections play). Or if you would like to be interviewed to take part in a project like The Indigo Collective.
