I understand and acknowledge that participation in any basketball activities, including practices, games, and training sessions, organized by the Vietnamese Basketball Association (VBA), involves inherent physical risks, including but not limited to sprains, strains, broken bones, concussion, and in rare cases, death.

2. Assumption of Risk

I voluntarily assume all risks associated with participating in VBA basketball activities, whether known or unknown, and whether caused by the negligence of VBA or otherwise.

3. Release of Liability

In consideration of being permitted to participate in the VBA programs, I hereby release and hold harmless the Vietnamese Basketball Association, its officers, coaches, staff, volunteers, Chicago Park Districts and affiliated partners from any and all claims, actions, damages, or losses arising out of my participation.

4. Medical Treatment Authorization

I authorize VBA staff to administer emergency first aid and seek emergency medical treatment for me (or my child, if under 18) in the event of an injury. I understand that I am responsible for all medical expenses incurred.

5. Agreement to Rules and Conduct

I agree to follow all rules and conduct expectations set by VBA staff and coaches. I understand that failure to comply may result in suspension or expulsion from the program without refund.
