Tranfers & Other Request Form- DPR Summer Camp 2020

This form should be filled out for any 2020 DPR Summer Camp Processing request.
Deadline Dates for Transfers or other requests:
June 1, 2020 for Sessions 1 & 2
July 1, 2020 for Sessions 3,4 & 5

Please note: Transfers and other requests are subject to availability. Camp registration transfers can only be made within a particular household to other family members.
