We are grateful to have your child participating in our Children’s Ministry this school year. Please fill out a separate form for each child. Information may be shared with the church's Children’s Ministry staff, contractors, and volunteers who work with your child.

Section 1 is required information; Section 2 is our Child Safety Covenant with required signatures from caregivers; and Section 3 is optional information, should you feel that it would help us to better know and serve your child.
