Prior to starting a new service-learning opportunity, students should complete this form to gain approval that the service-learning opportunity they wish to complete will count towards their service-learning hours earned for their service-learning award or diploma.
Once a student completes this form a copy is automatically sent to the school's service-learning coach.
After the service-learning coach reads this form, the service-learning coach will send the student an email letting them know that the opportunity qualifies for hours earned toward the service-learning award or diploma.
