All responses are confidential. Your individual numbers will not be shared with anyone outside the member of Wisconsin Clean Cities staff. Any data shared will be aggregate data that reflects the entire coalition. Please fill in each categories that best applies to your company. Year always refers to calendar year (January through December), not model year or fiscal year.
