I give permission for 2Sports to obtain emergency medical treatment, including emergency transportation, for my child if I cannot be reached immediately. I agree to be responsible for any emergency medical expenses incurred (If parent/guardian refuses to sign, instructions must be attached stating what procedure the facility is to follow in an emergency). The undersigned assumes all risk of injury or harm to the child associated with participation in 2SPORTS activities and agrees to release, indemnify, defend, and forever discharge 2Sports and its subsidiaries, vendors, staff, employees, and agents of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of death, injury, loss or damage to the child, however caused, arising by reason of or during the child’s participation in 2Sports. I hereby grant permission for the staff of this facility to have access to my child’s records. I hereby grant permission to share my child’s health information with emergency medical professionals.
I have read and understand this Youth Sports Program Participation Waiver, and I voluntarily sign it with the intention of being legally bound hereby.
