All individuals and organizations are required to pay for any missing, damaged, dirty, or wet items. For missing or damaged items, there will be replacement fees required for each item. Replacement fees for individual items are as follows: ~All Preserve dishware (plates, bowls, and cups) are $6.25/item. ~All silverware (forks, knives, and spoons) are $2.00/item.For any items that are returned unclean or not sanitized, there will be a $50 service charge fee applied. Rural Action staff will arrange payment with you upon receipt of this form.
***Cleaning removes food and other types of soil from a surface. Sanitizing reduces the number of pathogens on that clean surface to safe levels. To be effective, all items borrowed and shared need to be cleaned, rinsed, sanitized (very hot, clean water with sanitizer), and allowed to air dry. We recommend the following ways to achieve sanitation: ~Use 1 teaspoon to every 1 quart hot water in a clean sink and/or wash tub; soak cleaned items for at least one minute. ~Run all reusable dishware items through a dish washing machine.
