Preferences for your every month schedule. This will help me create a comfortable schedules that considers your needs as much as possible. Keep in mind, while I will try to accommodate your preferences, final schedules will depend on staffing needs and fairness among the team.
Before submitting your preferences, please read the following:
1.
Writing a preferences does not mean it is guaranteed at all.
2. This form is for expressing preferences only — such as your favored shift type (day/night), department (e.g. ward, ER, NICU3, PICU), or general scheduling pattern (e.g. prefer/avoid consecutive duty days, avoid split shifts, etc.).
3 This is not a monthly request form. Although You are free to change your preferences anytime its still not appropriate to change them monthly!.
4. Remember that some preferences might affect your schedule, be considerate about this point.
5. If you want to update your preferences, write them all in one paragraph and write the word (Update) so I can update them and delete the old ones.
6. Preferences are not guaranteed at all— every effort will be made to accommodate your preferences, but duty coverage and fairness will always be the priority.
Examples of preferences you can include:
• I prefer my duties to be all/mostly either day/night shifts.
• I don’t prefer Pedia Ward assignment.
• I prefer my duties mostly in NICU3/PICU.
• I would like to my duties to be consecutive, like ( 4 duties straight followed by longer offs even if its affects my weekends).
