Obligatory Registration form for  season 2020


FILLING THE FORM FROM HOME:
1. Fill out the form and hit submit. We will automatically receive your filled form at our manifest.
2. Once you get to the DZ, just report yourself at the manifest with your name and tell us that you have already filled out registration. We will print out your form.
3. You need to review your form, sign it and get a Gear check & DZ Briefing.
4. Bring the signed form back to the manifest and buy your tickets.

1. Fill out the following form and hit submit.
2. Once you finished, go to the manifest to pick up the printed version of your registration.
3. You need to review your form, sign it and get a Gear check & DZ Briefing.

TERM 1 *
I am in appropriate psychophysical condition: not under the influence of alcohol or drugs, medications that could impair my good judgement, and not under the influence of stress
TERM 2 *
The physician who performed my medical examination for skydivers as well as the authorized skydiving instructor are informed of previous (including cured ones) injuries or illnesses
TERM 3 *
I understand the task of the jump and I feel capable of performing it without bringing myself or others in danger and without causing material damage
TERM 4 *
On my part, the jump will be carried out in conditions in accordance with the legislation of the Republic of Slovenia and with rules and regulations of each respective airport

I certify, that my skydiving equipment is in flawless condition, properly maintained, and in conformity with the legal regulations of the country they have been issued by

I certify, that the insurance company I'm insured with for the case of damage incurred to a third party would cover the costs of damage cause by me during the jump.
