WVW Student Assistance Referral Form 2024 - 2025
The Student Assistance Program is a state mandated program designed to identify and assist students.  Students are referred for observable behaviors such as a decline in grades due to a circumstance out of their control, suspected drug use, signs of depression or withdrawal, and other behaviors that inhibit learning.  

Anyone may make a referral. The identity of the person making the referral is confidential and will not be shared with the student.

If there is a concern regarding the welfare of the student (a crisis such as self-harm, suicide, medication misuse, drug or alcohol abuse), PLEASE CONTACT A GUIDANCE COUNSELOR, PRINCIPAL, OR NURSE IMMEDIATELY so that home contact and interventions can be made as soon as possible.

*Academic Concerns, make parental contact first.
