You can include only your child’s name without any personal information (i.e., address and phone number) or you may choose to omit your child’s name and other information from the directory completely. We do, however, encourage you to let us print your child’s name so that we can publish complete class lists. The information filled out on the bottom of the form is what will be printed in the directory. If you wish to omit any information, simply leave that section of this form blank. In the event that you want your child's name AND all information excluded from the directory simply check the box indicating your choice and click submit at the bottom of the form.
In order to publish the Friend Finder on a timely basis, we ask that a parent or guardian from every family complete this form no later than Friday, September 7, 2018. Even if you do not wish to have your child's information included, please complete this form with your child's name and check the appropriate box below so that we know you have received it.
