I am healthy and under the age of 30. *
I have not been out of the country in the last 21 days. *
I have been practicing social distancing, washing hands for 20 seconds or longer, and interacting only with those in my household as recommended by the state of Telangana and other governmental authorities. *
I understand (and, if under 18, my parent or guardian understands) that by signing up as a delivery driver for Zoomers to Boomers, I assume the risk that I may come into contact with people who have the COVID-19 Virus or and that I or my household members or other close contacts may contract the COVID-19 Virus. *
I pledge that by signing up as a delivery driver for Zoomers to Boomers, I will follow all safety protocols in place by Zoomers to Boomers, as may be modified from time to time, to minimize the chance of contracting or spreading the COVID-19 Virus. *
I represent and warrant to Zoomers to Boomers that I am licensed to drive by the State of California or other governmental authorities.

I represent and warrant to Zoomers to Boomers that any vehicle I use in performing services for Zoomers to Boomers shall be either my personal vehicle or a vehicle I am authorized to drive.

If I, or any of my household members or other close contacts, while I am working as a delivery driver for Zoomers to Boomers, begin to feel any symptoms consistent with the COVID-19 Virus, I will immediately notify the team and quarantine myself for a 14 day period. *
I (and, if under age 18, my parents or guardians) hereby release, indemnify, defend and hold harmless ZoomerstoBoomers, its founder, director(s), and other organizers, from and against any and all claims, causes of action and liability arising from or in any way connected with my volunteer participation with ZoomerstoBoomers, including as the same may result of coming into contact with and/or the spread of the COVID-19 Virus. *
