Edge Fitness Freeze Membership request Form
If you wish to Freeze your membership please complete the following form

In order to process a freeze, please consider the following guidelines

Only members who are in good standing (no arrears or outstanding payments) can freeze their membership
We allow a maximum of 3 months per year to freeze a membership
Membership can only be set to Freeze in the following scenarios:
1. You are going overseas for work or leisure
2. You have a doctors note stating that you are unable to train and the duration for which you are unable to train
3. You are travelling for work

We require proof in the form of flight tickets with your name
Doctors note with reason and dates you are unable to train
Letter from work on company letterhead stating the term you will be working away for
