2025 Glad Run Sign Up and Waiver
Heart's 4 Amari is hosting the 2025 Glad Fest Run.  To participate there is a $40 registration fee that can be paid via Venmo @hearts4amari, on our website at www.hearts4amari.com, or cash the day of.

Date:  Saturday August 9, 2025

Registration Check In: 7:15am - 8:15am in the North Parking Lot of the Momence High School

Race Begins Promptly at 8:30am

I understand that participating in a running/walking event involves potential risks of
injury (including but not limited to: falls, injury, and/or death), loss/damage of personal property, contact with other participants,  effects of
weather, traffic, and road conditions. I voluntarily assume all such risks and agree to
participate in the Event entirely at my own risk.

I, on behalf of myself, my heirs, personal representatives, and assigns, do hereby release,
waive, and discharge Hearts4Amari, its organizers, volunteers, sponsors, officers, agents,
and affiliates from any and all claims, liabilities, or causes of action of any kind arising
from my participation in the Event.

Medical Treatment
I authorize Event personnel to provide or seek emergency medical treatment for me if
necessary, and I agree to be responsible for all associated costs.

Photo/Video Release
I grant permission to Hearts4Amari and its representatives to use photographs, video, or
audio recordings of me taken during the Event for promotional or fundraising purposes,
without compensation.

Fitness to Participate
I affirm that I am physically fit and medically able to participate in this Event, and I am
not aware of any condition that would affect my ability to safely do so.

If signing on behalf of a minor, I certify that I am the parent or legal guardian of the
minor participant and agree to all terms stated in this Waiver and Release on their behalf.
