As of the start of Spring term, 2024, the COVID request form has been discontinued.  Some supplies are still available, but must be picked up at Mail Services/Material Management Services.  Please see the excerpt below from the PSU Currently newsletter for more details:

As Portland State University begins the Spring Term, this is a reminder to restock any hand sanitizers, disinfectant wipes, or face masks for office suites, conference rooms, and department-controlled classrooms and labs. Supplies were purchased through the COVID Relief funds by the university, and are the last of the free supplies available to departments. Once supplies are gone, departments will need to purchase their own. Materials Management has 2-liter Purell hand sanitizer bottles, antibacterial wipe containers, and KN95 masks. 

All materials are free of charge and available to PSU departments. You can pick up these supplies from the University Services Building Mail Center front desk (USB 114) with a valid PSU ID.
