Family Members *
List the first names (and last names if different) plus the age of each other family member associated with this membership. You can omit the age or enter "Adult" for anyone 21 or older. Type it like this (using comma and semicolon as separators)...David, Adult; Sally, 12; Freddy, 8

Other Phones
List other phone numbers we should have on file for emergencies and/or the occasional text message regarding the pool (i.e. sudden closure). Type multiple numbers like this (no dashes, but use a semicolon to separate them)...9998887776; 9998887775
Other Emails
List the other emails we should include in the distribution list (our main way of communicating to you).
