Long Arm Intake Request
This form will serve as a waitlist for my services.  Please answer each question below with as much info as possible, then once you have completed this form please text me pictures of your quilt (front and back of quilt top and backing) and I will let you know by text when I am ready for you to mail your quilt to me.  I will quilt them in the order they are received at my address.
