By enrolling my kids in the Summer camp, I acknowledge and agree that participation involves certain inherent risks, including but not limited to physical activity, outdoor conditions, and interactions with others. I hereby release and hold harmless My Deen, its employees, volunteers, and their board from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant or to any property belonging to the participant, while on program premises.
In the event of an emergency, I authorize program staff to obtain necessary medical treatment for the participant and agree to be financially responsible for any costs incurred.
