This form is for HTMC members to sign up for an HTMC hike. It includes information needed for the Trailhead Volunteer and COVID-19 contact tracing.  

Completing this form indicates that you are an HTMC member who (1) agrees to wear a mask at all times and to maintain proper social distancing of 6 ft and (2) understands there is a risk of COVID exposure.  Please only sign up if you are certain that you can attend, as space is limited to 10 people (9 hikers + the trailhead volunteer).

Waiver: Because of natural conditions, human activities, or negligence, today's hike, travels and related activities present risks of death, personal injury, and property damage and loss.  The undersigned understands the above warning and assumes the above stated risks to him/her self and his/her children and wards. The undersigned agrees, in exchange for permission to accompany, travel or hike today with the Hawaiian Trail and Mountain Club, to waive and forego all claims and lawsuits which he or she, or his/her assigns (or interested persons or personal representatives as defined in the probate code) may have or start against said Club or any club officer (including the trailhead volunteer) for any person's death, personal injury, or property damage or loss suffered during, or as a result of the hike, travels, or related activities begun today. The undersigned has read the above and understands it and its significance. By signing this waiver the undersigned also agrees to the above Hiking Policies and Procedures and certifies that he or she is physically and mentally fit to engage in this hike.
