All students are required to submit this agreement form with a parent present. According to instructions provided, the student and parent will type their name and initials after the following statements.

STUDENT AGREEMENT (Student reads and types their name as their digital signature). "I have read the orchestra handbook and I understand what is expected of me as a member of this organization. I understand the importance of treating other students, my teacher, our classroom, and materials with respect. I understand that in a team activity, my actions and attitude affect the progress of my group and the opportunities available to the whole group. I will do my best to help create a positive, supportive environment for all students. I understand that I will be expected to keep up with regular individual practice outside of class time to maintain acceptable progress. PARENT AGREEMENT - CALENDAR (Parent types initials into space below) - "I understand that at this time there are no required extra-curricular activities scheduled for the orchestra program. I understand that as guidance allows, there may be required activities communicated to students and parents later in the year. I understand that if there is a required extra-curricular activity that involves any in-person participation, and my child is enrolled in Connected Learning during that grading period, I may request an alternate assignment for my child to complete for this activity." PARENT AGREEMENT - FEES (Parent types initials into space below) - "I have read the handbook section about fees and I am aware of the amount and the Oct. 1st due date of the required orchestra participation fee for my child. I understand that there may be optional activities later in the year that may require additional payment for participation. I will contact Ms. Harvey prior to October 1 if I anticipate any financial hardship in turning in the orchestra fee on time" TYPE PARENT INITIALS INTO SPACE BELOW TO AGREE: *
