CISP Academic Record Request Form
CISP Parent/Student Handbook states: "When requesting paperwork for visas, transcripts, college applications, etc., please allow 2 weeks for CISP to complete the requested paperwork" (page 30). When school is not in session, please factor in a possible longer wait period.
Report cards can be requested for grades 1-8 as applicable.
Transcripts: After the School Counseling Department processes your student's transcript for release, it is sent to the parents/guardians and student first to check for any errors. The sooner that the school counselors hear back from the family as to the transcript's accuracy, the sooner the school counselor can mail, send, print and/or email the high school transcript.

According to your answer in the last question, please provide one of the three following answers: A) If you plan to pick it up in person, who will pick it up and when; B) To whom should the transcript be emailed [you must provide name and email address of the recipient] ; C) Provide full postal address and if applicable, the name of a specific person with whom you have been in contact.
