1. Acknowledgment of Risk:  

I, the undersigned participant (or parent/guardian if participant is under 18), understands and acknowledge that participation in sports conditioning and basketball involves inherent risks and dangers, including but not limited to physical injury, illness, or even death. I voluntarily assume full responsibility for any risk of loss, property damage, or personal injury, including death, that may be sustained by me or the participant as a result of engaging in these activities.

2. Release of Liability:

In consideration for being allowed to participate in sports conditioning and basketball at the Briggs Street Property, I hereby release, waive, discharge, and covenant not to sue New School Leaders, Inc., the Briggs Street Poperty, its directors, officers, employees, agents, and any other affiliated parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or the participant, whether caused by the negligence of the releasees or otherwise, while participating in these activities or while on the premises of Briggs Street Poperty.

3. Medical Treatment:  

I give permission for New School Leaders, Inc. and Briggs Street Property staff to administer basic first aid or to seek medical treatment on my behalf (or the participant’s behalf, if a minor) in case of an emergency. I understand that I will be responsible for any medical costs incurred as a result of treatment.

4. Indemnification:  

I agree to indemnify and hold harmless New School Leaders, Inc., the Briggs Street Property, and any affiliated individuals from any loss, liability, damage, or costs, including court costs and attorney's fees, that they may incur due to participation in the sports conditioning and basketball activities, whether caused by negligence or otherwise.

6. Agreement to Terms:

I certify that I have read this document and fully understand its contents. I am aware that this is a release of liability and a contract, and I sign it freely and voluntarily. If signing as a parent/guardian, I acknowledge that I am signing on behalf of my minor child and that all terms apply to my child as well.
