I understand that Black Meadow accepts cash, Paypal, and Venmo. Hourly rate for custom work is $200 per hour with a $200 minimum. I understand that tattoos cost money, and large projects may take weeks or months of regular appointments to complete. I understand that any cost or time estimates given are approximate and may change based on my project, my healing, the regularity of my sessions, and my general pain tolerance.

I understand that, even though I’ve filled out this whole form, I may have to come to Black Meadow in person for a formal consultation/drawing appointment. At this consultation, if I make a tattoo appointment, I am required to pay a $150 non-refundable deposit via cash, Venmo, or Paypal. This deposit comes off the final session for this tattoo project. If I cancel my appointment or no-call no-show I forfeit my deposit.
