THIS IS NOT AN ENROLLMENT FORM TO SIGN UP FOR 4-H! THIS IS AN INTEREST FORM ONLY, SO WE CAN FIND OUT MORE ABOUT YOU TO FIND A CLUB THAT WILL FIT YOU INTEREST, AGE AND AVAILABILITY.  THEN YOU WILL FILL OUT AN ENROLLMENT FORM AND PAY SEPARATELY

Please fill out below to better help us place you in a club or to get involved .  If you would like to start a club, please let us know by calling 774-404-7020 or emailing Cathy at acampora@umass.edu. All you need is an interest, 5 youth from 3 families and 2 adults to start a club! 
Clubs are run by volunteers and meet where and when it is convenient to them so you may have to travel.
