2025-2026 Senior Sponsored Hours Submission
Fill out the following form every time you complete sponsored service. This form must be completed within TWO WEEKS of the event you are submitting for in order to be counted. If an event is missing from the list provided, please contact Lauren Sugra or Samaira Multani on Remind. You will not receive sponsored hours if your name is not on the sign-up sheet and marked as present at the event. 
Description of service must meet 400 character requirement to receive credit.

Describe the event in detail and how you participated to earn community service hours. Discuss the organization, your service, etc. Please be thorough. If you do not have enough to say about your performance, it is probably not worth service hours. *
