CMH Birthday Party Reservation Request
Fill out this form to let us know you are interested in booking a party with us. Reference our birthday party page for details. 

We respond to inquieries on a first-come, first-served basis. It may take a few days for our Events Staff to reach out to you as we have parties on the weekends and we are not in our offices. This does not affect your place in the queue, your inquieries are time stamped and are booked in the order they are received :)

We book parties generally 2+ months in advance of the date of the party. If you are interested in a specific date and party slot, please schedule in advance!

Your party is not reserved until we have received the $100 nonrefundable deposit. If your date and time can be confirmed with our Event Staff you will receive instructions on how to submit your deposit. If not, we will reach out to you with alternate times and dates that may work for you!

Please submit this form first in order to secure your spot in the queue.
