I hereby release and hold harmless Maize South High School, the Maize South Basketball Camp, its coaches, staff, volunteers, and affiliated personnel from any and all liability, claims, demands, or causes of action arising from injury, illness, accident, or loss that may occur as a result of my child’s participation in camp activities. I understand that participation in athletic activities involves inherent risks, and I voluntarily assume those risks on behalf of my camper. I certify that my child is physically able to participate in camp activities and authorize camp staff to seek medical treatment in the event of an emergency.
