Bark Incident Response Form
Instructions: This form is for staff or volunteers to report incidents of interpersonal or systemic oppression at Bark. Bark is committed to creating safer spaces for our volunteers and employees. A staff member or board member that is not involved in the incident will follow up and work with the appropriate people to respond.
You are welcome to submit a report anonymously. If you would like a Bark staff member to follow up with you, please include your name and contact information.
