Equipment Reservation Form SDFC West Valley
This form will need to be filled out in order to rent out any listed equipment from the West Campus SDFC. All equipment within this list can be checked out. 

The requesting party will only be subject to fees if any checked out equipment is damaged or taken/lost. SDFC staff will not be present during the event when checking out any equipment, it is up to the requesting party to manage and maintain any games and equipment that they check out prior to checking them back in. If transportation of the Lawn Games is needed for the event or activity, please email the contact at the bottom of this form 24-48 hours prior to your reservation. The SDFC West may be able to accommodate, but it is not guaranteed. If the SDFC transports the Lawn Games, it is the responsibility of the renter to notify the SDFC, via phone or in-person, when the event/activity has concluded so our staff may come and pick up the equipment. 

Reservations must be made 5 business days in advance, and cancellations must be made at least 24 hours prior to the selected reservation times. Lawn games cannot be taken off campus or utilized for multiple days without being dropped off and picked up again the following day from the SDFC.
