Parent Permission Needed for Child to Play at Home Course During Golf Class and After School 
Goal of this document is to ensure coaches and parents are equally aware of the location and activities of golfers during and after school hours.

My child or children have permission to play at their home golf course as an unsupervised club member during the golf periods (4th and 8th) and after school on days that the rest of the golf team is practicing at Crystal Falls (Mon. through Wed) or when the team is at at school during golf period (Thursday and Friday).                        *
By giving my permission for my child /children to practice at their home course when not with the golf team, I agree not to hold coach/school/school district responsible for my child's/children's whereabout's and activities when they are unsupervised and not with the golf team. *
This responses on this form will remain active for as long as your child or children are on the golf team at Leander High.
