Please note that the YEPP pilot program is an intensive support program for African - Australian youth in Melton, Wyndham and Brimbank local areas, who are at risk of engaging in criminal activity.

This form may take 10-15 minutes to complete.  
The purpose of this form is to obtain information to enable our staff to reach out appropriately to new participants.  

All information obtained remains confidential and will only be used for the purposes of providing support within the scope of the YEPP pilot program upon consent from the young person and/or their guardian/parent on intake.
