First District PTA Honorary Service Awards Form 2022-2023

Honorary Service Awards are designed as a unique way for PTA units, councils and districts to publicly acknowledge both individuals and organizations for outstanding community service to children and youth. All funds collected with this program are used exclusively to support the California State PTA scholarship and grant programs. The HSA program includes the Very Special Person Award, Honorary Service Award, Continuing Service Award, Golden Oak Service Award, and the Elected Official Honorary Service Award.

The applicant(s) being considered should be recognized for outstanding service to children and youth and not routine, assigned responsibilities or for retirement. Remember to consider those who work behind the scenes quietly and efficiently serving youth.

In considering candidates for the First District PTA Honorary Services Award recipients, we need your input. In the space below, please nominate an individual or organization and describe in full the contributions they have made for children and youth.

Award to be considered for:

  • VSP - Very Special Person Award
  • HSA - Honorary Service Award
  • CSA - Continuing Service Award
  • GOSA - Golden Oak Service Award
  • EOHSA - Elected Official Honorary Service Award
