PERMISSION TO ATTEND AND PARTICIPATE: LH Kids 2026 Vacation Bible School (VBS) will take place on June 10-12, 2026 from 6:30 – 8:30 p.m. The parent/guardian undersigned hereby understands the potential risks involved with VBS activities and give permission for your child(ren), to attend and participate in LH Kids 2026 Vacation Bible School hosted by LIGHTHOUSE CHURCH. CONSENT FOR MEDICAL TREATMENT AND HOLD HARMLESS: As the parent/guardian of the above-named student, I hereby give my consent for emergency medical care prescribed by a duly licensed professional. This care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of my dependent. I understand that all reasonable safety precautions will be taken by the leaders of this activity and that the possibility of an unforeseen hazard does exist. I further agree not to hold LIGHTHOUSE CHURCH, its leaders, employees, and volunteer staff liable for damages, losses, diseases, viruses or injuries incurred by the minor listed on this form.
