Tax Day Appointments

-PLEASE CHECK IN 15 MINUTES BEFORE YOUR APPOINTMENT IN THE FRONT OFFICE THEN GO TO ROOM 906.
-IF YOU WOULD LIKE TO CHANGE YOUR APPOINTMENT COME BACK TO THIS FORM AND PICK THE APPROPRIATE TIME SLOT OR CALL MRS. SEKHRI AT BONITA HIGH SCHOOL.

Please bring the following:

1)      Social security cards, Driver’s License/ID, routing and account number for refunds, tax documents like W-2 forms, 1099INT, 1099DIV, 1099G interest statements from savings accounts, stock dividends, a copy of the previous year's tax return and a pen to fill out a form.

BHS students please complete and bring the following documents too. (Print/ pick them up from Ms. Sekhri in Room 906):

2)      You can fill out and bring the following Intake Interview Sheet tinyurl.com/intakevita

3)       Student Free Tax Help Permission Slip https://tinyurl.com/accountingform (This form is only required for Bonita High School students)
