We offer two forms of payment: an hourly fee as a temporary employee or you may submit an invoice (and W-9 form) as a vendor. What is your proposed cost for offering the class? Which form of payment do you prefer? Please Note: All proposed fees are negotiable. 

Material fees may be included in the initial cost or may be listed as an additional fee to be paid directly to the instructor? Would you have any additional material fees? If so, what would the cost be.
