The camper listed above has my permission to participate in the AMS summer sports program. I hereby covenant and agree to release and forever discharge Manhattan/Ogden USD 383 its agents, servants, employees, and coaches, the USD 383 School Board and its members from any and all claims, demands, losses, damages, costs, expenses, and attorney’s fees for and health issues or injury to the undersigned dependent resulting from, growing out of, caused by, or arising in any manner out of playing or participating in summer sports and organized athletic activities at and for any Manhattan/Ogden USD 383. I certify that the participant is physically fit to participate. This Warning, Agreement to Obey Instructions, and Release is applicable to all summer sports and organized athletic activities in Manhattan/Ogden USD 383. By typing my full name below I agree to these terms.

The camper listed above has my permission to be seen on social media. I agree to the use of my child’s image, video, voice to be used to promote or represent the School District in news media and School District websites and social media pages. I agree to release and hold harmless the School District of USD 383, Board of Education, agents, officers, contractors, volunteers, and employees from and against any and all claims, demands, actions, complaints, suits or other forms of liability that shall arise out of or by reason of, or be caused by the use of the campers captured picture. By leaving this blank, student's image and achievements relating to camp will not be shared.
