This questionnaire evaluates Prakruti. Your prakruti, or natural constitution, is your unique balance of the doshas which was determined at the moment of conception. It represents you in your most balanced, healthy state.

More than one answer is acceptable for each question, however, it is best to choose the ONE option that is MOST applicable.

This is not a comprehensive Ayurvedic evaluation. Please note that while it provides a great deal of information, a full consultation is the best way to discover your constitution.
