Freshmen Applicants
  1. Original High School Report Card (Form 138)
  2. Original Certificate of Good Moralย 
  3. PSA Birth Certificate
  4. High School Diplomaย 
  5. Form 137-A
  6. Two (2) copies each of 1x1 and 2x2 pictures in White backgroundย 
Transferees
  1. Certificate of Transfer Credential with Documentary Stamp
  2. Evaluated Transcript of Records
  3. Certified True Copy of Form 137-A from last school attended
  4. PSA Birth Certificate
  5. Certificate of Good Moral
  6. Official Transcript of Records with the Remarks/Purpose: GRANTED TRANSFER CREDENTIALS with encoded purpose: Copy of Flight Dynamics School of Aeronautics, Inc.ย 
  7. Two (2) copies of 2x2 picture in White background
NOTE:
  • Required documents/requirements can still be submitted 1 week after class start but preferably be submitted before classes.
  • To secure your enrollment slot, the applicant may pay the RESERVATION FEE of Php 1,000.00 (this is non-refundable but deductible from your tuition fee upon enrollment) through our School Cashierย or via Online Transfer/Bank Depositย to the following bank partners below.ย 
  • Once paid, email the deposit or transaction slip toย acctg.flightdynamics@gmail.comย cc:ย fdsareg.cebu@gmail.comย with the subject "FDSA SLOT RESERVATION/ADMISSION 1st SEMESTER SY 2025-2026โ€,ย the email must indicate the Applicant's Full Name (Last Name, First Name and Middle Name) and the level/course/program being applied/enrolled.ย  You will receive yourย ย Official Receiptย ย thru this email.ย 
  • ADMISSION FEEย is Php 1,000.00, provided the applicant has already made the above Reservation fee (this is deductible from the tuition fee) - this promo is valid for the first 50 students only.
