Borang aduan/cadangan ini disediakan untuk menerima sebarang aduan daripada warga/pelajar/orang awam. Dengan adanya borang ini, urusan dengan Laboratory Animal Care Unit dapat dicakna dengan lebih efisien.Setiap aduan/cadangan yang diperoleh akan diteliti terlebih dahulu. Tindakan penyelesaian aduan/cadangan mengambil tempoh 1 minggu.Pentadbiran LACU tidak akan melayan sebarang aduan yang menggunakan bahasa yang kesat, lucah, konfrantasi, penghinaan atau tuduhan tanpa asas.Pentadbiran LACU juga tidak akan campur tangan dalam kes aduan yang melibatkan keputusan mahkamah/badan arbitrari/seumpamanya serta mana-mana pertikaian yang melibatkan masalah peribadi antara syarikat/individu yang tiada kaitan dengan peranan/fungsi agensi kerajaan.Sebarang pertanyaan mengenai aduan yang telah dibuat, sila hubungi:Pentadbiran LACU : +603-6126 5130 / 5132 _____________________________________________
This complaint form is intended to provide a medium to receive any complaints from the public. With this system, it is hoped that the public affairs with the Laboratory Animal Care Unit will be further simplified.
All complaints will be investigated first. All suggestions and complaints will be acted upon within 1 week.
The Administrative of LACU may refrain from handling any complaints that use abusive, obscene language, confrontation, insults or baseless accusations.
The Administrative of LACU will also not interfere in complaints cases involving the decisions of any court / arbitrary body / similar as well as any disputes involving personal problems between companies / individuals that have nothing to do with the role / function of government agencies.
